I have a bearing with a sleeve with dimensions as per attachment.
The dimension inbetween brackets is a non-important dimension.

a standard bearing 17/40/12 mm has as part number 6203, but that's not what I need.

Anyone knows where I can find the part number for the bearing as per this attachment?

Applied.com
got it within few seconds of searching. bookmarked that site!

Is the sleeve tapped with set screw like a pillow bearing is?
Mine aren't, but the replacement I've seen on applied.com is. The threaded holes don't really bother, but due to clearance issues, I will have to remove the set screws.

Are you sure that you don't have a cam loc hub on the bearing? Look closely at the ID and the OD of the hub and confirm if they are concentric or not. If they are not concentric then it is a cam loc hub.

Being belgian, I didn't quite get all that.

What is a cam loc hub?
Is hub the same as sleeve?

ID and OD of the sleeve are concentric.

Above is a link to a cam loc hub in the Timken verity. A lot of people will confuse the extended race bearing with a cam loc.

I believe what you are calling a sleeve is an extended race bearing http://lutco.com/Products/Radial-Bearing/Radial-Extended-Inner-Race-Bearing/

Indeed, mine are extended inner race bearings. However, unlike the one in your second link, mine don't have set screws...
