Old Craftsman double sided socket?

Has anyone ever seen something like this?

It's a Craftsman USA V-series, has a regular 12pt opening, but the drive side has no detent grooves and has a secondary set of openings cut, similar to an 8pt socket. It doesn't stay attached to any of my ratchets (everything from Kobalt to Snap on).

Thoughts?

Looks like a factory oops.

Yep. It might be too late to exchange for another USA one now. Hurry, look through the store stock for a USA one or else you will only get China. A working China socket would be better than an unusable USA one tho.

Looks like a factory boo-boo or some sort of owner modification.

I looked online and didn't see anything like it, so I thought the knowledge base here would be able to say whether or not it was something goofy.

What strikes me as odd is the machining quality. I'd definitely believe it was a factory error.

I'm with the factory oops theory.

It looks like the 3/8" broach hit it twice, and the second broaching was not exactly 45 degrees from the first either.
