Old Reznor Heater CO issue

I just discovered this forum... first post! We moved into our current house 9 years ago. It has a detached shop/garage with 2 car bays. A reznor propane heater came with the garage. We never tried to use it until last week... mostly because at some point our house was completely converted to electric and so we never turned on the propane at the huge tank near the house for just that one heater. Got the pilot lit and it fired right up blowing warm air. We used it for about 2 hours then shut it down and locked up for the night. The next day I called our local utility company and had them come out to check it out for propane leaks just to be safe. No propane leak, but he said the CO level was high and not to use the heater until I got it fixed or replaced. The weird thing is... when he came out to check it, the heater had not been run that day at all. The last time it ran was about 24 hours prior when we ran it for a couple of hours. So, I was initially thinking must be a cracked heat exchanger, but then I thought well maybe the vent pipe could be clogged and the CO kept leaking out even though the unit had been off for a while. Today, my son and I got on the roof and took off the vent cap. We removed a couple of old wasp or hornet nests that had most of the pipe (80%?) blocked. I am hoping this was the problem and I can use my heater again, but I will first call and get the CO level checked again. I have searched online about reznor heaters and CO leaks but did not come up with much at all. Anyone know if cracked heat exchanger is common with these heaters? I have not found much evidence of that but I am sure it must happen. Do you think the clogged vent was probably the problem?
Are there any other scenarios that could be the issue with the heater. Also, just to note... nothing else has been run in the garage... not power equipment, tractor, etc. so the high CO level must have come from the heater right? Thanks for any help or advice on these types of heaters... I know very little about them. Speaking of that... what is the black screw for in the closeup picture I took and attached below.

Blocked vent would certainly cause an issue, that type is a natural draft vent so it will have limited safety features to shut it off if the unit isn't venting properly.

That flathead screw is covering the adjustment for gas pressure.

It wouldn't be a bad idea to have that unit professionally cleaned, inspected and combustion tested to insure safe operation.

A CO alarm wouldn't be a bad idea.

Thanks for the advice. Yes CO alarm will definitely be installed. The gas pressure adjustment is interesting... can that have an effect on CO output? Do I adjust it to get as pure blue flame as possible... is that the point of it? Thx.

Do not adjust it without using a manometer at the minimum setting gas pressure to whats called for on the rating plate( with LP it's most likely going to be 10" water column) or better yet using a combustion analyzer. You cannot adjust it by eye accurately.

Here the utility company employees condemned every gas stove the checked because they didn't check it right. I would get your own CO detector and try it to see what you are getting. The one I have shows the PPM of CO so it should be close to as accurate as his meter. The correct method on stoves as approved by UL and Gas agency was a process that measure the air free CO out of the stove vent after stove had been on for 15 minutes and could be as high as 800 PPM air free direct from the vent. His waving his detector over the stove and condemning the stove and leaving a red tag was an issue on several occasions and required showing customers the UL instructions for testing stoves not the utility companies attempt to read CO. All gas appliances and gas flames make CO when the gas flame hits cold steel/pans but once they warm the steel, the CO output changes to CO2. Even a cracked heat exchanger once it warmed up could vent CO2 not CO into the building.
