Got this at closing auction for local garage and towing company, in business since 1955.
Marked Patrick-McDermott Co, Los Angeles CA.
Never seen anything like it.
It’s 3/4 drive and original owner told me he changed a lot of truck tires with it, back in the day.

Here’s a close up of the mechanism.
Swing big weighted lever which screws itself into engagement with a dog.

No intention of selling mine...that’s thing works amazingly well.

I have a wrench very simular to that.... I gave $20.00 for it at a yard sale a few years ago with a set of 3/4 drive sockets....
A friend of mine had one many years ago so I knew what it was....
I always assumed they were military....
Never knew a name for it, just called it a slugger wrench.....

Just a quick flick to engage the spinning handle?

Has to work either direction for RH & LH lug nuts.
