Proper install of a HRV?

What is the appropriate way to install a Heat Recovery Ventilator?

My house has one currently, and I'm finishing the basement and must move it and redo much of the ductwork and seems like it may not be properly installed.

currently there are 4 ducts to it, one draws fresh air from outside, one vents stale air to the outside, one is duct'd to the return air duct near te HRV, the last is duct'd to the return air duct aswell, but this one is about 15 feet away from the unit.


I'm thinking it should be the way it is but the fresh air to the house be tied into the supply duct, unless the HRV is only suppose to run while the furnace fan runs, then i would say the fresh should be tied to the return, and the stale to the supply.
