I've never seen one, so they can't be very common. Still, these were give-aways and thousands may be buried in the bottom of old tool boxes!
I would imagine they were made in the Canadian plant in London, ON. I can't imagine why production would need to have been in the USA...
Obviously, there may have been a large number of "General Hardware Distributors" over the decades - and a quick search for Toronto yields nothing - but if it was a business located in Toronto, it had to have been after March 1956 as that is when the "LE" exchange (LEnnox) (really called "Central Office Names") was introduced.
