Maybe the clamp is not tight or badly worn.

On my Milwaukee, the clamp and screw are considered a wear part. The clamp flattens, the screw strips. Cheap parts, I bought several spares last time.

If your blade clamp is like that one the problem is going to be the parts required are now discontinued / considered obsolete. The Clamp Pin 648594-00 can still be had but the Retainer Kit 884696 looks to be unobtanium.

EDIT: The vid says you can't reuse the old retainer kit (roll pins) because it will just vibrate out but perhaps a tack weld might fix that? IDK. If so it'd be a one-time fix though.
