I am looking for about a 5" round plate with a hole in the middle to cover round electrical boxes on the ceiling (and the cut in the osb). The plate screws right on to the electrical box. The hole in the plate would be to run the wires through to wire in my four foot florescent lights. The lights themselves don't completely cover the round electrical boxes. Any ideas would be appreciated. I actually found one in a Home Depot video but can not find it at any Home Depot or other lumber yard stores.

Menards has them with strain relief for the cord, or with a knockout to run wires through. Check around.

They all are white and typically sit on peg hooks with the ceiling fan parts.

The covers are for what is called an octagon box and any good electrical supply should have them. Called octagon box cover.
I got some from my local building supply when I was doing the same.
The flourescent that used to cover the box had to move so I used one of the covers to do exactly what you want to do. My garage had one 4' flour which was totally inadequate so i added 3 more.

If you can't find one with the hole already just use a hole saw to cut the hole. Just be carefull that things can't spin out of control.
