the wire is old and the sheathing is falling apart.

It's usually the power company's responsibility, but not necessarily always. Go to their website and look around, there may be a 'blue book' or installation guide available there. Or, just call them and ask.

Usually the OH drop upto the splices @ the weatherhead is the PoCos and from there down is yours. In the case of underground, its usually the PoCos...

Recently upgraded my home to a 200 Amp service.

Power company, San Diego Gas & Electric, installed new service drop to my new weather head and mast.

I bought the panel, mast, weather head, and the wires within the mast. They also supplied a new meter.

Here in Ohio with AEP, it's the same, the PoCo is responsible to the crimps where the incoming service line attaches to the weatherhead/mast/ service enterance.

PoCo . . . . that's why in ice storms when trees fall on aerial drops to houses, it's responsibility of PoCo to replace. Makes sense due to safety factor.

If the mast on house got tore down, then it's homeowner responsibility, along with wire down mast to meter and service wire to panel.

Around here the meter and meter can is also 100% PoCo (must be installed my master electrician but paid for by PoCo).

Easier to get advice from strangers around the world on the internet than actually talk to someone...

I like to have an understanding of the correct answers before dealing with $10/hour phone call takers.

As a general rule, PoCos are responsible for the wire and equipment to the designated service point. Customers are responsible for the wire and equipment beyond the designated service point. Most often for homeowners, the meter is the service point. In that case, the PoCo will be responsible for the meter but the customer will likely be responsible for the meter base (not always). Many PoCos will have this information on their website but it can be hard to find. The OP may have no choice but to call.
