Looks like self taught teacher was giving himself a crash course on what not to do on a lathe. School of hard knocks.

Workpiece wasn't balanced yet and wobbled off the centers.

That mount point certainly didn’t look like it had much surface area...I always get nervous during that part of turning...and ALWAYS where a full face protector...
