Found this cleaning up, its an older underlined Snap On replaceable face hammer but it's been hammer on itself and I can not make out the model. Looks like a "B?-232" stamped on it.

also check the handle for the 3rd hammer face, sometimes it screws on the bottom of the handle
Likely Nupla made that hammer, or if not , the handle and grip. I have several S-K (I assume Nupla made since Nupla is stamped on teh base of the grip handle) that are identical except with the same fiberglass shaft but a green handle. Larger ones like 32oz and 40oz have teh same handle and shaft but in black rubber grips and Nupla heads. Or at least those that i own.
Same number of grip slots as well.
Those are NLA and remember a lot of older hammers out there have replacement handles. I have a S-K head 16z ball peen with a Nupla handle and grip identiclal to what you show but with the green grip but it looks like a replacement.
Most of the profesional users do not care about exact replacements. They need a handle quick. The crib mgr. fixes the tool. Done
