SO Warranty Registration Card

I just recieved a Snap On CT6850 and was going mail in the warranty registration card and ran into a problem. Someon placed the model/serial# sticker over the line under the addres, I have no idea what info goes in this slot _ _ _-_ _ _-_ _ _ _ (I can make out a CUSTOM under it) Any help?

Just call customer service; they can help and probably register it over the phone...

they also want your city state zip
phone number and date of purchase. just fill that in at the bottom and it will be alright.
