Can somebody help me, what's the name of this tool?? what is this tool was used for??

Sure looks like a tube, or conduit bender to me, dedicated to one size.

Looks like it might be a shear for rebar or other type of rod.

Are there matching holes on each side and what is the diameter?

Not much travel with the short section of teeth.

Just a guess

Yes, now i can see, it has a matching holes on both sides, 3/4'' diam.
Makes sense, its a very old shear.
