Hopin the garage types can help me out.

What would you call a bolt that looks like this? The page refers to it as an eye bolt but I'm not finding any other pages that have eye bolts with the eye no larger than the width of the bolt itself....

without the nut it looks like the stud the goes with the sling swivel on one of my rifles, if it had a ring it would look somewhat like a ring bolt.

Don't know if this helps any.

we use them to attach the chains to the access gates at work. I ordered some a couple years ago, can't remember what they're called though.

It's an eye bolt, just not the cheap hoop style. You would use these where you have a clevis pin link at the end of a chain

It depends on the application-I would call this one a lifting ring as it is approximately 6" in length, 1.5" I.D. by 2.5" thread x 7/8"-good to lift a large portable air compressor or generater or air conditioner of approximately 5000 lbs. by a crane to roof tops.
