So now that I have the picture thing figured out.

My 02 Mountaineer has this rust issue pictured below, seems to be a common issue on these bodies. From what I have figured out the 'outside' patch seems straightforward enough, I was wondering how to go about doing the part on the inside that the outer skin get tacked to.

Saw that you already read through the "first patch panel" thread. If you haven't already, read through every page of MP&C shop Projects. Take your time and absorb the info. Almost every page can teach you something!

Is the inner piece rusted out as well? It may be simply working from the inside out if not. Just Cut out the inner fender as well and weld a patch in there so that you can tack in the outer panel.

From what I can tell, the inner part will need some work also. How much I have no idea yet as I'm still not to the point of tearing into it yet. Thank you for giving me another place to look for tips and info!

Get some similar sheetmatal. I like to use part of the roof or trunk lid from a donar car.
You will mark the area that needs to be replaced. Remember that the area to be replaced will be bigger than you think as the metal around the rust hole will be too thin to weld to.
Use a hammer and some kind of form ( pipe, anvil, dolly, railroad track, old car rim, etc) to hammer the metal on to make it the shape you need it to be. Bigger pieces or complex shapes will need to be welded together from several smaller pieces unless you are really good at metal shaping.
Remember to make things as clean and rust free as you can and prime inside areas before closing them up
