Top flange bolt?

Hello! I got a message from a coworker that has us stumped. He had to cut off a bolt and is trying to find a replacement but we can't figure out what its called to find one. Its like a carriage bolt or elevator bolt but has a hex neck. Its flange-hex-threads. Below are pictures of the flange portion of the bolt as well as where it goes. It comes up from the bottom with the hex in the opening and the flange holding it in place so you can bolt the next piece down.

Any ideas on what this would be called?

That somewhat resembles an elevator bolt. But judging from that heavy metal component I am guessing it is not some Ace Hardware off-the-shelf part. Who made the machine/attachment from which it was removed?
Also, did you check McMaster Carr?

That hole is hexagonal?

I'm guessing it got disassembled wrong (hence the cutting) and that's some sort of press in PEM stud

Since OP says nothing about threads not sure how much closer I can get

If this is a 1-off, I would take the appropriate size bolt (thread wise) and mod the head as needed to fit the hole. Then weld a thick washer to the top of the head.
