Sounds like a depth perception problem to me. Does she wear glasses, if so maybe an eye doctor appointment would be in order.

As a side note, a few years ago she bought a new car with the back up camera, and front an rear sensors. Day #1 she leaves the house and 30 seconds later she walks back in with a weird look. I said, "why are you back already?" She said, "I backed out of the garage and into a car parked in the street." Dumbfounded, I asked, "why didn't you STOP when you heard the sensors beeping?" She said, "I thought it was the seat belt alarm because I hadn't buckled yet."

Yes this is what I'm dealing with, for better or worse.

you are not alone with these issues. I should have run when we were dating.
we went on a camping trip, her and a friend went into town and came back at night.

she pulls up close to the camper to be close to the camper to unload. smart gal, except she parked the car in the lit camp fire.

no the car did not burn up, a few of us were sitting by the fire with our mouths agape and yelled at her to back up.

^^^Yes, you know exactly what I'm dealing with!!!^^^

These are all great ideas, and I've considered most (except the rubber chickens :) ), but my idea was specific to retrofitting an actual traffic light. I think I'm going to play with the Arduino project ideas (in addition to the standard parking blocks). I'll post updates on it when I get around to making it.

I did the tennis ball thing with a twist. I hate the ball in the way when the car is out of the garage. The string is hooked to the top panel of the door and there are two eyes on the ceiling the string goes through.

One is at the place where the car should stop, the other is placed so when the door is up the ball is at the correct place. It takes a few tries to adjust it but it works well.

Here's a few pics, one is a drawing of what the string looks like when the door is open.

I don't know what it is with some drivers. My wife has more trouble backing into our garage. I wanted her to learn to back in, as she used to go to work in the dark, so she would have run all over the lawn backing out in the AM. She's been backing into the same garage for almost 6 years now, and she still winds up at a 30 degree angle sometimes. I put a mark on the wall, and showed her how to watch it in her side view mirror. Her new car has a back-up camera. Doesn't seem to help. She marvels how I can place the car so perfectly even though I rarely drive it. Thinking ahead, I pinned a concrete curb to the floor so at least she doesn't wind up in the back yard. She drives perfectly fine unless she has to back up.
