Toe nail puller.However, in OP's picture, the tool seems all metal, and the puller arms have pointy hooks. I doubt it's for medical use.
My guess: it's for pulling on a cord or spring

OMG, it’s a torture device.
IMO, The OP’s has way too big of a center post to pull gears on most clocks I’ve been inside (2). Maybe the clock from the mOvie Hugo.

Good one! It's indeed a toe nail puller.
Wondering how long that thing should stay on in order to bend the toe nail to the desired shape. Can't be 5 minutes and done kind of deal.

Whenever I see this thing I cringe, then picture some guy sittin at home with his feet up watchin the game. Then he gets up to get a drink/snack and catches it on the carpet/couch/ottoman/dog/etc...

Most of those pop up ads on FB are scams from China. Beware. I know this first hand. I found a similar puller at an estate sale over the summer. It was from clock collector. Those pics give me the creeps.

I see the problem from the guys that are wearing narrow toed high heels in the shop. If you do that then order one more tool.
