walker1.5 ton floor jack

I have this old walker 1.5 ton floor jack that needs seals. how do i find what the model number is? I've been told I need the jacks model number to get the correct seal kit. I've attached pics of the jack with the pump assy removed. the jack will pump up as long as there is no weight on it, as soon as weight is present it will not lift

Looks like the metal tag might be on the base of the handle assembly, but appears to be a J122/93630 or early 32. The majority of suppliers assemble universal kits to cover all the variations between those models, at least I did and I know HPS in my signature does. I would recommend a tear down and inspection of the Jack prior to forking out cash for a kit, just in case something is awry internally.

I finally got the jack apart. most of the seals are cracked and broken. the plastic piece at the end of the main ram is stuck at the bottom of the shaft. does that piece come with a seal kit? is this a problem (meaning not worth repairing)
