Large diameter hose connection. 4", etc. think fire hose. The connections have what are called "rocker lugs", that the "hook" part of the wrench hooks into allowing for more leverage. Hand strength alone is not often enough to get a good seal or loosen the connection before or after the hose has been pressurized.

Two are used in opposing directions on the male and female ends of the connection point.

Just needed one last week to remove/install a nut on a power steering pump gear for a Cat Diesel. Also used allot on hydraulic cylinders.

For spanner nuts with square slots. Nuts with holes on the periphery use a pin spanner.

I'll second the use for hydraulic cylinders. These style wrenches are mostly for low torque. Usually on hydraulic cylinders they end up slipping and I am forced to use a brass bunch and sledge to break the gland nut because the wrench won't hold enough for the needed force.

When I worked on textile machines, I think I recall using them on some Terrot circular knitting machines.

This.....look on any fire engine and you'll see plenty of them.

Meat grinder

I worked in a butcher shop during high school/college and had a similar wrench but it was at least 24" long. The hopper you would put the meat in was huge.
