Does the handle have dimples in it. If so it might be a Ken Tool hammer. Show some pictures of the handle.

Yep.

It's a soft face rubber /nylon tipped hammer.

The nylon and rubber tips, one on each end, are missing but replaceable.

The square "nubbin's" hold the tips on.

Agree with above, it is or was a soft face hammer. Measure the size of the drive squares. Craftsman is usually the cheapest place I have found to purchase the tips. The entire hammer is only like $11 so no sense in sinking more money than a new one is. :beer:
