Looks like a torque/angle gauge, to adapt to a standard 1/2" drive bar.

It's a torque gauge. You put a bar through that hole to hold it steady, socket on the square drive and use a breaker bar in the female square drive on the top. Instead of being click-type it's got a gauge and you use a bar to apply force and a reaction bar to hold it steady.

I picked up a Snap-on version in 12/ dr. for $25 a couple of yrs ago. Looked a snappy site and they don't even list 1/2 inch anymore.

It's nice ecause you can use it at the end of an extension and cancel out the twist of the ext.
