Looks like a hole saw for tile backer board. One carbide tooth, a rudamentary hole size measuring device, could be.

It looks to me as a tile hole cutter, my conclusion is based on the fact that there is a carbide pilot drill and carbide point on the end which is adjustable. There is still a variation of this hand drill still produced if my memory serves me well.
