Is that top piece supposed to be included in the kr660 unit or is it like an accessory add on?

What do you mean by "top piece"? You mean the top box? No, they were sold as separate boxes, although they made have sold packaged sets. They are a nice box size and layout IMHO, although many today won't like the plethora of shallow drawers, especially at the top.
