It looks like some mushrooming was removed. Possible a tool struck with another hammer to start a hole for setting large spikes before driving them home.

Yes, I ground down the mushrooming. In the 3rd picture, you can see a hole for some sort of round tool, similar to the one on the other end. Evidently that end fell out at some point, and that end got used and mushroomed as a hammer.

Sorta looks like a Geology or Paleontology Hammer, but a much older version of what you'd see today. Maybe something used for mining back in the day?

Or, just keep it as a "watsit" wall hanger guaranteed to be worth its weight in interesting comments and maybe deserving of a new hickory handle. I have a section of wall in the garage dedicated to items worthy of interesting discussion or speculation.
