I found this in a box of tools in an old barn I was cleaning out...what is it. My initial thoughts are a spring compressor, but I'm not 100% certain. Any guesses

Hand milker for cows before they got electric milkers............LOL Some kind of spreader from the way it looks.

Just google "antique valve spring compressor". I'm pretty sure there's a pic of that exact same tool.

I'm pretty sure you need to take a closer look at the OP's photo. That ratchet arm doesn't compress, it EXPANDS the jaws. Also, a spring compressor would have a small lip to keep the spring from sliding out of the jaws.

Looks like you are right. Maybe for a larger engine?
