I have a big maxx 45k heater and i vented it out the back of my garage with 3" b vent. It faces the north so when its really windy the flames shut down inside than relight again. I am assuming the exhaust is blowing back into the unit so it is turning the flame off. If i put a 90 elbow with a 12" vertical piece than a cap do you think that will help? It will be about 2 feet from my siding and three feet under my eave. Or could i just put a 90 than a cap on that?

Actually if you are using metal B vent it should go up over your eave . The 90 and some pipe and a cap will help with the problem you are having .

do you think I can use 3" all the way up, on a 45k big maxx, it will be over five feet if I go over my sofit and above my roof line?

Most manufactures recommend that the flue exit to _ feet (a specified amount) over the tallest point within _ (a spec amt) feet or inches please refer to the owners manual for the specs. Also a special exhaust termination is specified. this is offered only as a opinion.
