    OK, I think I found it. I am pretty sure it is a Forward Lift 9000C. The manual is from 1995, but if you look at manual for the Forward Lift 9000FP it has a better schematic of the lift. My money is all in on it being the 9000C model!

    I found this BP-9 that sold for auction. Yours has very unique baseplates, and the main columns are different as well compared to the BP-9. If your serial number starts with an MM, the BP-9 on the auction site appears to be numbers only. Im super curious to see what it ends up being.
