I did not manage to solve it either. I had the tap wrench replaced. I think the issue results from non symmetrical jaws.

How is the quality of the Izar Tap Wrench? With the last one I bought, I found an issue with the jaws, they did not align correctly which caused the tap to not sit straight. Are they still made in Spain?
