First piece of advice - buy all your parts from monoprice - you'll save a ton of money.
Also agree on the conduit for future proofing. I assume you arleady have power at the TV source, so we're just talking other cables?
Since this is a new project, I'm assuming you are installing a relatively new TV (HD flat panel).
Use HDMI since it carries audio and video in 1 cord - easy to run. I'd run two, since most AV receivers aldeady do HDMI switching. The two will cover your dish receiver and dvd player.
I would also run an IR repeater. This would allow you to completely box in or otherwise hide your components (dish receiver and dvd player) but still be able to control them by pointing the remote at the TV. These are somewhat expensive, but useful if you want to keep everything hidden.

HDMI wall plate (for a clean look) $10.95 each. Place one behind the TV mount location and another at the component location. Use the In-wall HDMI cable to run between these.

Regular HDMI cable (for the component to wall and TV to wall connections) - $3.56 each. And to think some people pay $50 for the "monster" version of these at local stores... for shame.
