Good luck with your surgery. Those online forms are worthless same thing happened to both my wife and I on recent procedures. No one reads them and they instead call usually at dinner and ask all the same questions then when you arrive you have to fill out forms with all the same questions. So frustrating.

I got that call today!
I got frustrated last visit as the questions asked in person, were the same as online and as asked by phone
I solved the problem with a Word document that has all my medical information. I print the first two pages (one sheet) before going to a new doctor/dentist/facility and hand it to them. It has my name, date of birth, address, phone numbers, email, insurance numbers and pharmacy (name, address and phone) at the top. Next is a table that lists all the doctors I see or have seen in the past, with their specialty, phone and fax. Next is a table that lists all the medicine I take or have taken, with the generic and brand names as well as the dose and frequency. The drugs I no longer take are shaded (with an explanatory note above the table that no one seems to notice). Finally, there's a table that lists all the major procedures I've had done with year, location and doctor.

Sometimes they ask "When was your last visit with your [fill in specialty]?" so I started tracking current treatment with dates and description of each doctor visit. When I get "that call" I offer to fax the two pages of information (most medical facilities still use fax machines) or the whole package. Those calls take very little time now and faxing or handing them the information three or four times is quite satisfying. Really cuts down on the form filling time at the office -- the answer to every question is "See Attached."

Bob, that's a great idea, I will try that.
I went to another Dr appt at 10:00, this should be the last one before surgery! I caught two errors on my meds already, the first was no big deal, they were double dosing me on an Anti-inflammatory, the second could have been deadly, another case of the left hand not knowing what the right is doing.
The surgery center called, be there at 3:15 tomorrow afternoon, remember dont eat anything after midnight tonight!!! I told her she had to be kidding, she said that he had a lot of surgeries to do, I said I know I'm one of them.
I have been typing this since 10:00 this morning, I keep getting interrupted and have had a bucket load of things to get done. The surgery center called back at 3:30 and told me to be there at 2:00 I told her that it was cruelty to a fatboy! She just laughed, I was being serious!
The chickens dumped about a 5 gallon bucket of feed onto the ground last night so I made a piece and riveted it to the door.

The nest box wasnt finished, only the bottom six were usable, I cut six bottoms and installed them, now all 12 boxes are useable.
I did a lot of cleaning and other things but I wont bore you with that.
Had pork chops and taters for supper, not sure what Pat will fix a midnight but she said she would fix something!

Well, I really appreciate all of the well wishes, they are really important to me, thank you all.
Its 11:30 I have stocked the chicken feed, changed the water filters in the house, hauled the garbage off, I think I'm ready to go. We'll leave here at 1:00, to get there by 2:00. I'll report back here when I'm able. Thanks guys!

I made it!!!
I am actually walking without assistance, the Doc did a great job.
I am carrying a cane with me but I fed the chickens, went to the shop and charged a battery for the bil, I dont have to do pt. Doc said, see you Feb. 6th.

Awesome news. Sounds like everything went by the book or better. You'll be back at it in no time!

I'm doing very well, the Doc said no pt.
Funny story, they called me about the time to leave and said there had been a fire in an A/C unit and the center was closed down.
I was to go to the hospital and check in there.
I lost a couple of hours waiting comfortably in a bed with a tv on the wall

Happy to hear that everything went as smoothly as possible.
I have a motto for myself, break easy but heal faster. I was much younger when this held true.
I think I am smarter now. So, I take less chances. At least when the wife is not looking.
