I don't know where y'all get the idea that you aren't supposed to roll an engine hoist with a load on it. How tf do you get an engine out of a car or back in? 🤦
He is right those wheels are useless with a heavy load. 300-400 lb might be all right but go past 1500-2k that thing wasent moving on smooth concrete without a come along. I'll try better casters.

Oh I know they are junk. 2k caused one to shatter for me. I adapt pallet jack wheels to mine so they roll easily.

If you look at the shop crane pictures, it's pretty beefed up. I'm hoping to lift 5500lbs 3 inches off the ground to slide a mobile base under a lathe. Pretty confident I can handle it, I seem to worry more about this crappy hydraulic ram that keeps leaking as soon as I bought it.

My excaliber engine hoist boom buckled right above the ram with 3000# on it at the 3000# extension. I don't know what you added to yours but it looks like just angle. A truss adds a lot of strength, essentially making the section as tall as you want for vertical strength. Also, where you extended toward the sides, where the legs used to attach, that really needs to be beefed up as it's going to want to pull the middle down. Boxing that so it's essentially a continuous tube from one side to the other will help there. Mine lives outside and the middle tube filled with water and froze. So I replaced it with a heavier tube and added tubes for the bolts to go through so they can be cinched right without crushing the tube. It made it a lot more rigid though I don't know if it really benefits as much from it as far as strength goes.

Where are you saying to put the truss?

Build a crate for it onncasters, lay it back far enough to get through the door, then set it where you want it and remove the crate. Shouldn’t be too hard, and the crate will keep it stable.

Same place most other engine hoists have them. Also, put the vertical stand off directly above the ram. I see a few where it's not there and think to myself, clearly not a ME that designed it.

You wonder if that or adding quarter inch plate to both sides of the boom would work, or possibly both put the trust in and then sandwich it in between two pieces of 6" plate?

I'm already going to change everything on the base. It bent when the load shifted. Go tomorrow I'll cut it out and replace it with 4x4 quarter inch square tube

I would rent either a forklift or a Sky Trac that is rated for that weight. It's likely above 2000 pounds.
