Click for the album. You can see the folded rear corner which guarantees that the tool can't get pushed through the back. You can also see that the belt-grip is bent to conform with my side. I wear a Wilderness Instructor's belt, 1.75" width, with a Kydex reinforcer inside (the CSM model).
I basically use a heat gun and a home-made press (yoga mat on 2x12) to form the kydex, then wrap it around an old belt with several layers of cloth wrapped around it (so the belt grip isn't too tight). There's a specific order to the folds, but it's pretty obvious once you start doing it.
