Sorry for the late reply, but this is how the slides come out on my box.
The plastic tab on the right gets pushed down

The one on the left gets pushed up

While pushing both of those tabs at the same time, you should be able to pull the drawer out of the cabinet.
On the drawer side, this Phillips screw needs to come out, then you can remove the slide from the drawer. There's one Phillips for each slide

Next, on the inside of the cabinet, work the slide until you find this Phillips screw towards the front and remove it. Again, 1 Phillips per slide.

Finally, take a punch or flat screwdriver and lightly tap to get the bent metal tabs out and remove the slide
