I may have misunderstood and my apologies if so. I thought your were trying to mount a plate at the surface.

I was thinking something like coupling nuts and a frame buried in the concrete

Yes, buried in new 12" thick concrete, with steel beams going two feet deep into sonotubes below the new 12" slab. Rebar per spec, 6" keyin under the perimeter of the existing slab, pinned to the existing slab, and anything else the concrete man recommends.
