i found a couple screwdrivers, a wrench, and a $20 bill when i took the bottom drawer out.

I'm not afraid of a little metal work. I was honestly just curious as to how/if SO would do anything or if anybody has had this happen before.
I'll probably end up making some as you suggest. I'll still need a couple slides however.
The slides themselves are the least of your problems, those can easily be obtained from Snap On. They are still produced.

3 of the slide guides on the cabinet are screwed."
How many drawer "levels" are in need of help? What is the overall condition of the box? If you have the space, especially since you have the top I would put the effort forth to get it fixed.
Do you have an overall picture of the boxes? The price was definitely right.

According to my driver you can order the spot welded guides, for the boxes I couldn't get a part number as I don't know what box you are working on

I would bet SO will still have the parts for you to fix the box. Maybe not the drawer, but they may have a listing for the complete drawer.
SO sells a lot of products that have serial numbers on them. They also include a registration to SO. For many years it was up to the buyer to fill them out and send them in. Several years ago my dealer started filling them out before the tool ever left the truck. I would guess at some point he sent them in.
I know he sent many in for me. Once I included an email address, I have had emails from SO thanking me for my purchase.
I have had only a hand full of dealers in 35 years. As time has went on SO record keeping has gotten better. The dealer I have now actually has the records from the last two dealers I have had. I am not sure how they come buy this. Whether SO provides them or maybe it is in the dealer contract that they provide them to the next one. Maybe they have some kind of way to purchase it from a dealer that is quitting, I do not know.
Proof that you are the original purchaser may not be that hard, if you are. I would guess that SO has dealt many times with people that are not, and have a pretty good system of figuring that out.
In my case, I have a receipt for every SO tool I have purchased new. For the last ten or so years the dealer has taken care of the product registrations. I only sent a few in before that. I also know that my dealer can go back about 24 years on my buying history.
