Water heaters are allowed to have a permanently installed loto hasp in residential applications, so are dishwashers.
LOTO wasn't required for either on my house but that was 20 years ago. The water heater in the shop didn't require it either and that was 3 years ago. Although that "inspection" was kind of a joke. He spent as much time talking about my nice floor finish as looking at electrical. Is that a thing in latest electrical code? I'm not too worried. Only my wife and me and she doesn't mess with the electric panel - if there was a problem she would ask me about it.

The interlock for a vertical breaker would have to be different than the simple slide for a horizontal main. Wonder how they do those...

The actual requirement is for permanently connected appliances over 300 VA, or 1/8 hp, to have disconnecting means. that can be either a disconnect at the appliance, or permanent LOTO provision, or with line of sight of the panel and less than 50'. It's been in the code a long time, it's in my 2011 copy, and not changed there. Earlier this winter, we stayed in a 2002 build condo in Idaho that had loto provision for the water heater, garbage disposal, and something else (not labelled). (I don't normally look at rental electrical panels, but the hot water ran out much earlier than we thought it should, so I checked for a trip).
