What size head did they have? a standard 1/4 20 x 5/8 has a 7/16 head and a thin wall socket will not fit into the access hole. I am guessing that a 3/8 head pan type head is what I need. Like I said I never saw them. Thanks.

Something feeds both sides of the box. Usually a main breaker output has two legs, one feeds each side of the panel. I have had main breakers located by the meter panel fail on one leg that has caused similar situations. Start at the source and work your way forward.
