So, how are the Plvmb ratchets, honestly? Serviceable, or just a good looking piece of steel?
They're just as good as the Proto version.
The Plomb pear heads came in two styles--round knurled handle (early), or pebble-finish handle (late) like the one on the flare-nut wrench you just got. The knurled part of the early handle is smaller diameter than the Proto knurled handle. Which one people like best is a matter of opinion.
From my experience, Plomb/Proto pear head ratchets rarely need anything but cleaning and lubing to be completely serviceable.
Can the inner workings be updated to finer or more teeth?
No. Honestly, they work fine the way they are, unless you're working in really tight quarters where you're barely able to move the handle.
If you're someone who only uses fine-tooth ratchets, you probably won't like them.
Some of us who have been using Plomb/Proto pear head ratchets for years think they're the greatest thing since sliced bread.
I've got a couple of fine-tooth ratchets, but I only use them in tight quarters. The rest of the time, I reach for a Plomb/Proto pear head or my old Snap-Ons, (which are also not fine-tooth ratchets).
