    Ouch… I’d be inclined to weld it up, drill, and retap at this point. That would have been a bolt that threaded into the threads in the hole - therefore not needing a nut.

    Typically you figure this out before you start and put equal amounts to either side. Depending on how the spacing works out, you either figure with a sheet in the middle or a joint in the middle and space things out from there.

    Re: Rebar can cause shrinkage cracking There’s your problem… I wouldn’t go any more than 12” o.c. for such a slab. Also looks like it cured too quickly and wasn’t kept damp long enough.

    ^^ This. Calculating out a beam is a lot different from calculating out things for a gantry crane. I’m thinking there is a reason why your last setup collapsed… No idea where you are, but building codes likely have something to say about it as well.
