I’ve installed thousands of feet of rubber base. Go with the standard glue on. The self stick is much harder to install, because you can’t massage it into place, it sticks; then falls off later. The self stick fails to stick half the time. I really don’t know why it’s even made.

Helicoils are kept in place by springloaded tension. The coil springs outward and grip the parent material.

After seeing the photo, I would tap it; or weld it full and tap it. Not much meat there for threaded insert.

You may very well be able to tap the hole for an M6.5; rather than Helicoil or Timesert, both of which will leave less parent material.

Thicker concrete doesn't hurt anything except your wallet, so grade or don't. Let it dry before pouring, as the clay base will shrink when it dries from the water you put on.
